The Body Align Clinic are the data controllers who will safely and securely store your personal details as part of your medical records, this information is not outsourced to any another third party, nor is a data processor used to control this data. Lewis & Charlotte Wood are registered as a Partnership and are trading as the Body Align Clinic.

You have the right to request restricting the use of your personal data e.g. contacting you via telephone or email, sending text/email appointment confirmation, receiving online exercise software (MyPhysioRehab) or for marketing purposes from the Body Align Clinic only. Your personal data will be used as part of public health records, which is a legal obligation. Please note: If you wish to unsubscribe from our database you will no longer receive appointment text reminders or email booking confirmation.

Our lawful basis of storing data is based on a legal obligation to comply with article 6 of the General Data Protection Regulation (GDPR). As we provide a private healthcare service to our clients, this health information is recognised as being special category data. Therefore we ensure that this data is processed according to article 9 under c) Processing is necessary to protect the vital interests of the data subject or of another natural person where the data subject is physically or legally incapable of giving consent.

As part of signing our Client Registration Form, you have provided us with written consent to discuss or share your medical records with other healthcare professionals, including General Practitioners, Consultants, Therapists, Medical Imaging Clinics & Fitness Instructor, etc. Any confidential documents will be posted recorded delivery or emailed as a password protected pdf file.

All medical notes will be securely stored for the recommended period of 8 years for adults (age 25 for children), after this period these records will be continued to be stored by the clinic whilst trading (incase required by the individual or for any medico-legal reason). Individuals can provide a written request to access their medical records. This information is required to be kept up to date and accurate, therefore please inform us if your personal details have changed. All data is held in the UK and not stored outside the EEA.

Written consent will be requested if any photo and/or videos taken as part of a treatment session are to be used as part of the client’s medical records and securely stored by Body Align Clinic. Please be aware that we use CCTV video footage (without sound) in our car park and reception area for security purposes and for public safety. Any video footage taken is kept for a one-week period only then re-recorded over.

Consent for children under the age of 16 will still be required from whoever holds parental responsibility for that minor to attend a healthcare appointment and use their personal data as part of their medical records.
